Restoration of registration

If you wish to restore your entry in the register then you will need to complete an application for restoration.

Completed forms can be returned to us by email or post.

The current fee’s are:

Pharmacists

Restoration following removal for non renewal - £486 (£102 application fee plus £384 restoration entry fee )

Restoration within one month of the date of voluntary removal- £486 (£102 application fee plus £384 restoration entry fee for restoration)
Restoration after one month of the date of voluntary removal - £267 (£102 application fee plus £165 restoration entry fee)

Pharmacy Technician

Restoration following removal for non renewal - £297 (£102 application fee plus £195 restoration entry fee )
Restoration within one month of the date of voluntary removal - £297 (£102 application fee plus £195 restoration entry fee)
Restoration after one month of the date of voluntary removal - £120 (£102 application fee plus £18 restoration entry fee)
