If you're not sure if you need to declare a caution or conviction, our guidance may help.
Use the form at the bottom of this page to tell us about any issues which may affect your fitness to practise if you are a pharmacist or pharmacy technician registered with us, currently in training, or applying to join our register.
You can use this form to tell us about issues including:
- cautions or convictions and other criminal offence outcomes
- investigations or fitness to practise proceedings with other regulatory bodies
You do not need to declare fixed penalty notices for offences such as speeding.
If you have already declared a matter as part of your application to the register or your annual renewal, you do not need to declare it again unless something has changed.

What you need to give us:
- as much detail as you can about the issue you are declaring
- details of your employer or training provider, if you have one

When you have submitted your declaration, you will receive an email to confirm that we have received it. This will include a reference number which you can use if you need to contact us about your declaration.
Once we have reviewed your declaration, we will be in touch to let you know about the outcome and any next steps.
