Re-registering a pharmacy premises

Under the Medicines Act 1968, if you want to re-open a pharmacy and it has been less than 12 months since it was last registered, you can re-register the pharmacy by completing a pharmacy premises restoration application form - (on click the document will attempt to download automatically).

If the pharmacy premises has not been registered for more than 12 months, complete a new premises registration form

Completing your application

Make sure you read the guidance that comes with the application form before submitting your application, to help you provide all the information we require, in the correct format.

As part of your application, you will need to complete an assessment to show how you will meet the standards for registered pharmacies [PDF 985 KB] from the first day your pharmacy is open. 

If you plan to operate a pharmacy service over the internet, you will also need to show us how you propose to meet our guidance for registered pharmacies providing pharmacy services at a distance, including on the internet [PDF 365 KB].

Submitting your application

We accept application forms that are either electronically completed or handwritten and scanned (as long as the writing is clear, in black ink) and saved as a PDF.

We accept supporting documents saved as GIF, JPEG or PDF files.
