If you wish to restore your pharmacy premises to the register within 12 months of the date of removal you are required to complete our Premises restoration form.
The application fee for restoration is £106.
A restoration entry fee is also charged:
- For premises removed following an application for voluntary removal: £135
- For premises removed for non-payment of the renewal fee: £714
If you are applying to restore pharmacy premises to the register more than 12 months after the date of removal, please complete our New premises application form.
Please be advised that if you send your application via post, we recommend that you send your application by registered post and track delivery to ensure it has been delivered.
