We regulate pharmacists, pharmacy technicians and pharmacies in Great Britain to keep you safe and give you the confidence to use pharmacy services.
We work to assure and improve standards of care for people using pharmacy services. Our role is to protect patients and the public and give you confidence that you’ll receive safe and effective care.
One of the ways we do this is by investigating concerns about pharmacists and pharmacy technicians where there may be a risk to patient safety, or where public confidence in pharmacy could be affected. We also look into concerns about pharmacies. We carefully review and assess all concerns before deciding whether we need to begin an investigation.
To report a concern you need to fill out our online form

What we will and won’t investigate
We only investigate serious concerns about pharmacists, pharmacy technicians or pharmacies, where there may be a risk to patient safety or public confidence in pharmacy could be affected. Find out more about what we do and don’t investigate.

Reporting a concern for someone else
You can raise a concern on behalf of a child under 18 if you’re their parent or legal guardian. If you’re reporting a concern on behalf of someone over 18 years old, we’ll need to have their consent for you to act on their behalf. This should be a letter or email from the person you’re acting on behalf of, giving you their permission to report the concern. Please upload this in the attachment section of the online form.
What information we need from you
Please give as much information as you can when you complete our form. This will help us to quickly decide if we need to investigate, and the best way to begin our investigation.
You’ll need:
- the name of the pharmacist, pharmacy technician or pharmacy you want to report a concern about (if you know this information)
- the date (or rough date) that the incident (or incidents) happened
- details of the incident – such as the place where the incident happened, what you feel was wrong, and what happened as a result
- details of anyone else you have reported this concern to
- electronic copies (for example, scanned images or photos from your phone) of any documents connected with your concern. You’ll be able to send these to us later if you don’t have them now.
You will now be able to save your progress on the form and come back to finish and submit it later. There's a 'save your progress button' at the bottom of each page; click on it to save and follow any prompts the form provides.
